Please use the link below to register your child(ren) for enrollment at MTSD. Please carefully read each section and include as much information as you can. We look forward to seeing you!
You will be asked to upload the following documents during the registration process:- Student Birth Certificate
- Proof of Residency - acceptable documents are a copy of the current lease, mortgage statement, tax bill
- Immunization Record
- Current Physical
Please provide as many of the above as you are able to at the time of registration. Any documents that you are currently not able to upload will need to be emailed to Mrs. Heymann (heymannd@mansfieldtsd.org) and Mrs. Cosme(cosmem@mansfieldtsd.org) in our main office.
